Payment for treatment is due at the time of service. I currently do not participate as a contracted provider for any insurance company, but am considered ‘out of network.’ Many PPO insurance plans may partially reimburse for services treatment. You may request a detailed receipt for each visit, called a superbill. You can submit the superbill to your insurance company for reimbursement. Reimbursement varies widely and is dependent on many factors including your deductible, out-of-pocket max, and rates set by your insurance company.

Rate: $300/hr with sliding scale
